Independence Care Center is part of the Independence Health Systems' group of high-quality elderly care centers. We are dedicated to meeting the needs of the elderly and are a not-for-profit home.

Nestled in 13 beautiful acres across from City Park in Perryville, MO, we're conveniently located just off Highway 61. You'll find our lovely grounds are fully equipped with sitting areas, a courtyard, and a gazebo.

Your Full-Service Nursing Facility

Independence Care Center features a 133-bed, skilled nursing facility which includes an Alzheimer’s dementia unit called “Creative Care”.

The rehabilitation philosophy that we promote encourages residents to remain as independent as possible. As a team, we'll work to provide opportunities that maximize the full potential of each individual.

Semi-Private Rooms: $200.00 per day
Private and Deluxe Private Rooms: $223.00-$235.00 per day
Creative Care Rooms: $207.00-$223.00 per day

A sincere thank you to everything you did personally for mom and our family. Thank you Sherry for providing the recliner for mom to sleep in, the coffee and breakfast for us. You went above and beyond. You will forever be in our heart. All the nurses and CNA's  were awesome. The night nurse when mom passed was amazing. My dad truly enjoyed the time the nurse took to talk and comfort him. I know he was busy but still took the time. And thank you to housekeeping and dietary, you all were wonderful!

It’s important to know that there is someone in your corner. Hospice offers emotional support services. Hospice is a wonderful asset to Independence Health System. When you choose for your loved one to go on Hospice, it simply means you have chosen to focus on the quality of your loved one's life for however much time you have left. The goal of Hospice care is to reduce physical pain and create emotional well-being.


We are blessed with the best Hospice Care Groups. Journey Hospice, Legacy Hospice, Crown Hospice MO,  Gentiva Hospice, Serenity Hospice Care, and Mercy Hospice Cape are the Hospice groups that provide services at our facility. And we couldn't do it without them!! We are thankful for all they do for  our residents and families.
